Medical Records
Your Right to Access Medical Records
You have the right to request and receive copies of your mental health records. This includes:
- Diagnoses
- Treatment plans
- Progress notes
- Test results
- Assessments
When Access May Be Denied
We may deny access only if a licensed provider believes releasing the records would:
- Put you or someone else in danger, or
- Cause serious harm to another person named in the record
Response Time
We will respond to your request within 30 days by:Â
- Providing the records, or
- Letting you know if they can’t be found, or
- Explaining why access is denied (if applicable)
Fees
We may charge a reasonable, cost-based fee for:
- Paper, CDs, or USBs
- Labor for copying
- Postage if mailed
- A written summary (only if you agree to it)
We do not charge for searching or reviewing records.
