Referral Process

REFERRALS & ADMISSIONS

The Pavilion admits patients 24 hours a day, 7 days a week. Our admissions coordinators are available to discuss patient history and medical necessity, arrange admissions, facilitate transportation, schedule evaluations, and assist with insurance verification. While referral sources can make the direct call to our admissions department, individuals may also self-refer to Pavilion or be referred by family members, physicians, community services boards, interventionists, and private therapists.

ADMISSION CRITERIA

Admission to The Pavilion requires the patient to be medically stable and appropriate for an inpatient level of care.  When contacting the admissions office, please have the following information available:

  • name, address, and phone number of prospective patient
  • date of birth and social security number
  • extent and nature of the problem, including areas of concern and information to assist in treatment planning
  • insurance Information
  • name, address, agency, and phone number of referral source (if applicable)

We encourage inquiries to be made during business hours (8:00 am–5:00 pm) in order to ensure verification of benefits. Once confirmed, the admissions staff will arrange a convenient date and time for the patient to be admitted.

Pavilion has relationships with many managed care organizations and insurance plans. We explore every opportunity for payment options with the individual and his or her family. Current contracts include:
